Maybe genetics (highly inbred) or more likely parasitic.

My guess is gill flukes. Quite common (your fish grows up with it until gills seem permanently asymmetric), not serious until the fish surface to breathe, or one side totally shut and the other flapping like crazy. Easiest way is to give it a salt bath (30 g/L, 45-60 mins, common table salt will do) . Osmosis kills them before it kills your fish. Better way is to get formaldehyde (formalin: probable carcinogen. Careful!) base solution. The cocktail from interpet treating white spot has some. Or if you can get a hold of formalin (35-37% active), dose 15 ppm, permanent dip, 3 days. Treats tank env as well, but more water change until bioactivity returns. Get a syringe and some latex gloves from guardian.

The nitrifying bacts are not there/establish yet. It is first ammonia (dangerous= should be 0 all the time), then nitrite (dangerous), then nitrate (100-1000 x less dangerous), then nitrogen (which will not happen in your tank, that's why water change to remove nitrate).

Water change until filter media establish. Then water change when nitrate accumulates.

More of a flagellate. More complex, multicellular. Small enough to migrate within body systems via blood streams.
