Extremely swollen anus

Need help with one of my arowanas, guess ive neglected them lately

after a week of not inspecting them , i found one of them has extremely swollen anal area

anybody experienced this? I feed them with my last ti,e with shell and head, but removed the horn. Any advice? Planning to move later to hospital tank and dose with yellow powder and also epsom salt.

Would appreciate tips. Thanks
