such incidents dont happen overnight.
it is likely internal infection due to neglected care. or a very bad injury. anyway, to help it recover, you got to isolate the fish in another tank with clean water. provide aeration to keep the water rich in dissolved oxygen. some salt in the water will be helpful to the fish in this case. use something to support the fish to keep it in an upright position. dose some anti internal infection medication and hope it recovers no feeding until it is able to swim upright on its own. water conditions is likely to be bad in the isolation tank, so u got to do very regular water change to help the fish recover faster.

No a pro too. But once every month or two is not enough for arowana. What is the size of your tank and your fish?
