I'm not an expert. But it's most likely water issues.
You might want to provide more specifications of the tank so the experienced keepers can give a better advice.
Example:
Tank size
Type of filter
Temperature of water
Type of anti chlorine used and dosage amount
How much salt do you add? Etc
